Required documents for newborn passport application:
1. Child's birth certificate (PSA);
2. Passport-sized photo with a white background (make sure your ears are visible);
3. A photocopy of one parent's passport + proof of residence (Blue Card/i-Card/Visa Paper);
4. Marriage certificate (if applicable);
5. Parental consent form (template available in the App).
Replacement of passport/travel document materials (if lost):
1. Old passport/visa paper/residence permit/card (if any);
2. Passport-sized photo with a white background (make sure your ears are visible).
* Long-term visas can be converted into passports; tourist visa are limited to travel documents.
* After your passport/travel document is replaced, you will still need to apply for a visa before you can leave the country.
